POSITION SUMMARY
The Case Manager provides services such as screening, support, brief intervention, collateral services, training, education and guidance to youth and families. Strength-based and trauma-informed support services are provided at various locations including in-home, at schools, and throughout the community.
ESSENTIAL DUTIES
1. Provide behavioral management interventions to youth with trauma who are placed in foster care/adoption placements.
2. Provide in-home, in-community independent living skills training to youth and Non-Minor Dependents (NMDs).
3. Provide case management assistance, including but not limited to scheduling medical/dental appointments, creating school and work schedules, etc.
4. Document and communicate needs to the assigned Social Worker and participate in treatment meetings (e.g. CFTs, IEPs, TDMs) and discharge planning decisions.
5. Provide youth/NMDs transportation to school, appointments, supervised visits, or court as needed.
6. Supervise visits between foster youth/NMDs and birth family members and complete accompanying documentation.
7. Provide crisis intervention services to youth/NMDs and family members as needed.
8. Coordinate services and supports in conjunction with county and/or NMDs and Resource Parents and birth family members, including but not limited to coordination of therapeutic services, educational needs, family contacts, and other related activities.
9. Maintain a flexible schedule to assist with emergency and/or urgent situations. Overnight/weekend assistance with clients may be required.
QUALIFICATIONS
1. BA/BS degree in Behavioral Health or Social Science (or related discipline) and 6 months’ work experience OR Associate Degree in Social Science discipline and 1 year experience working with youth; OR equivalent combination of education and experience.
2. Knowledge of child welfare, behavioral health, education, and/or criminal justice systems preferred. Experience working with youth strongly preferred.
3. Possession of a valid California Driver License and current auto insurance.
4. Ability to pass/complete employment clearances through FBI & DOJ, TB test, etc.
5. Strong oral and written English language skills essential. Spanish bilingual skills preferred.
