PSH Case Manager

We are hiring a PSH Case Manager to join our growing non-profit! NWPP is a fun and supportive organization that provides meaningful impact within the Portland community.

Salary: $56,600 Annually

Job Duties:

- Maintain a caseload of approximately 15 very low-income seniors who have experienced chronic homelessness.

- Work collaboratively and respectfully with clients to develop effective housing plans to address immediate and long-term housing and healthcare needs.

- Meet regularly with clients, primarily in community-based settings and always with a strengths-based and client-centered approach, to assist with access to resources and services that support their housing and health stability.

- Provide assistance with housing applications, advocacy with landlords, move-ins into permanent housing, and/or eviction prevention services.

Job Qualifications:

- Minimum of one year experience providing social services to adults from diverse backgrounds and cultures.

- Interest in working directly with people who may be experiencing a variety of challenges, including poverty, aging, homelessness, and significant health issues.

- Willingness to engage in meaningful training, self-examination, and action directed at promoting diversity, equity, and inclusion in the organization.

- Excellent interpersonal skills.

- Experience working with computers, databases, and commonly used software.

Preferred Qualifications:

- Demonstrated understanding of the root causes of homelessness and unique challenges faced by seniors of color and other historically marginalized communities

- Demonstrated ability to provide culturally responsive services to vulnerable populations

- Bachelor’s degree from an accredited college or university is preferred

Northwest Pilot Project requires its employees to have or receive the COVID-19 vaccination and the booster shot. Our staff is currently working a hybrid in-person/remote model but that may change based on COVID conditions. Technology and equipment needed to effectively work remotely will be provided.
