Care Coordinator (Haitian Creole)

Care Coordinator - Haitian Creole Lasante Health Center is seeking a Care Coordinator to join our team. The ideal candidate will monitor our patients’ medical treatment and facilitate communication with healthcare providers to guarantee access to high-quality medical care. Responsibilities:

Regularly monitor and manage a designated group of patients with chronic medical conditions.

Enroll patients in different medical programs

Engage in consultations with patients and their family members to address health concerns.

Provide comprehensive education to patients regarding their medical conditions, medications, and specific care protocols.

Assist patients in scheduling appointments

Complete other tasks as directed by supervisors.

Qualifications:

Customer service experience

Computer skills

Ability to handle multiple tasks concurrently

Care coordination experience preferred
