Job description
Acti-Kare is seeking a compassionate and detail-oriented Client Care Coordinator to help seniors live independently and with dignity in their own homes. As a Client Care Coordinator, you will ensure our clients receive the highest quality of personalized, in-home care and support.

Key Responsibilities:
Serve as the empathetic point-of-contact for clients and their families, building relationships and addressing any concerns with care and sensitivity
Conduct thorough in-home assessments to fully understand each client's unique needs, preferences, and goals for independent living
Collaborate closely with our team of experienced caregivers to develop and regularly update personalized care plans that prioritize client comfort, safety, and quality of life
Oversee the scheduling of caregiver visits, ensuring continuity of care and monitoring for consistent, high-quality service delivery
Communicate proactively with clients, families, and the care team to provide updates, address challenges, and coordinate care seamlessly
Participate in the onboarding and training of new caregivers, providing mentorship and guidance to ensure they are equipped to deliver our signature brand of compassionate care
Thoroughly document all client interactions, care plans, and other relevant information in our electronic records
Assist with general office duties, such as answering phones and collaborating with other care team members
Qualifications:
High school diploma or equivalent, Bachelor's degree in healthcare administration or related field preferred
CNA or HCA certification preferred
Minimum 1 year of experience in home care, case management, or a similar client-facing role
Exceptional communication and interpersonal skills, with a demonstrated ability to empathize and build trust with a diverse range of clients and stakeholders
Strong organizational skills, attention to detail, and capacity to manage multiple priorities and tasks effectively
Proficient in Microsoft Office applications and adaptable to learning new software systems
Valid driver's license, great driving record, and reliable transportation to conduct in-home visits
Must be willing to drive in areas of King county, Snohomish county, and Tacoma areas to visit clients.
