Care Manager for Home Care Agency

SYNERGY HomeCare Olympia Open Management Position. SYNERGY has an open position for a Home-Care Manager. This person meets with new clients and establishes a Care Plan to provide care for our senior clients. She then meets with our other Managers and Scheduling to determine which caregivers would best match with the client. Other activities are introducing caregivers to clients, and help assess changes in condition of our clients and updating the Care Plan. Support the agency caregivers with training and resources. The position needs a compassionate and detailed oriented active caregiver manager. You will assist and provide care to our awesome clients acting as a liaison between the client and our agency home care office.
