The team consists of 8 RNs working with 5 NPs and 16 MDs. Responsible for delivering professional nursing care to patients via in-person, phone, or patient portal. Utilizes strong organizational and leadership skills to assess patient status; implements clinical nursing interventions to modify symptoms; develops, implements, and evaluates patient care plans and patient education materials to enhance and improve outcomes; and ensures a safe and therapeutic environment.This position is hybrid and requires an on-site presence at least three days per week. Job location is at the Intermountain Primary Children’s Hospital Miller Campus located at 2250 North Miller Campus Drive, Lehi, UT 84043.ResponsibilitiesEssential Functions0.80 FTE :

Performs phone management with patients’ parents, chart review, care coordination, triage, and problem management.

Discusses patient and family concerns in person/over the phone and/or via the electronic health record.

Provides and develops patient/family education materials surrounding common Gastroenterology disorders; communicates education best practices to proactively answer common questions to reduce future messages.

Communicates adjustments in treatment plan to families.

Formulates care plans for specific patients and documents the nursing process in patient’s record.

Evaluates patient response to interventions and modifies plan of care as needed to achieve health care goals. Communicates adjustments or changes in treatment plans to patient/family.

Develops and participates in educating patients and their families about care programs.

Communicates lab & other test results to families.

Provides medication refills.

Assists providers and staff with patient-related paperwork such as letters of medical necessity, DMV forms, school care plans, and preauthorization.

Maintains a safe and therapeutic patient care environment.

Works closely with triage team and gastroenterology staff to provide high quality, efficient care to patients and their families.

Keeps current on advances in nursing care by participating in educational programs.

Orients and educates new staff in nursing techniques and departmental procedures.

Utilizes knowledge of medications, procedures, infection control, and safety precaution, and initiates appropriate interventions in crisis situations.

Participates in total quality management to positively affect patient and system outcomes to improve care.

0.20 FTE :

Coordinates closely with our Inflammatory Bowel Disease ( IBD ) Clinical Nurse Coordinator to move these patients along the care continuum.

Works with the prior authorization team to ensure a seamless flow of patient care for IBD patients.

Ensures proper care coordination of IBD patients when the IBD Clinical Nurse Coordinator is out of office.
