Position Summary:Assess, plan, implement and monitor patient nutrition while actively engaging in providing nutritional education, information, services, and guidance to advise patients on food, diet, and nutrition by developing treatment plans.Position Key Accountabilities:
Performs nutrition screening, evaluation and assessment for patients.
Counsels program participants on basic and/or high-risk nutritional need based on certifications criteria and determines follow up continuity of care.
Provides medical nutrition therapy to meet patient goals.
Develops educational material and tools and teaches nutrition education classes following the class module and schedule as described in the Annual Nutrition Education Plan.
Markets, promotes, and raises awareness about nutrition services through communication with area health professionals.
Charting and keeping records to monitor patient progress.
Conducts and ensures clinical nutrition documentation for patients is accurate, informative and timely, including assessment for new admissions as appropriate, reassessment, food/drug interactions, and specialized nutrition support as needed.
Monitors industry trends, new products, current research, and new programs in medical nutrition therapy and food service and incorporates into his/her work as needed.
Participates in nutrition seminars, workshops, etc., to ensure continuing education in various areas of nutrition science.
May assist with teaching activities in diabetes education program.
Develops additional nutritional programs to address needs of patient population.
May assist with projects including but not limited to data collection and patient recruitment.
Performs other duties as assigned.
