EDUCATIONAL/TRAINING REQUIREMENTS:

High school diploma, some clinical training /BS in health related field preferredEXPERIENCE REQUIREMENTS:

One to two years of experience in a medical setting, Cardiac Surgery and/or Vascular preferred.

ADDITIONAL INFORMATION:

Some formal clinical training (Medical Assistant, LPN)

Ability to problem solve and perform multiple tasks simultaneously.

Proficiency in computerized scheduling.

Familiarity with office equipment use and maintenance, i.e., fax machine, copier, printer.

Excellent communication and interpersonal skills.

Customer service orientation.

Excellent organizational skills and attention to detail

PRIMARY FUNCTION:

The primary duties of the patient navigator are to coordinate the care of patients in a university-based outpatient specialty practice. This role includes working as a liaison between patients, physicians, and support staff to ensure delivery of safe and efficient care. Ensures coordination of patient follow up and potential family member appointments. Responds to all phone calls. Prepares patient packets for new patient visits.

ESSENTIAL FUNCTIONS:

Coordinates appointments for all Aorta Center patients.

Answers all phone calls.

Submits authorizations for all relevant testing and/or non-par authorizations for Aorta Center patients.

Prepares New Patient Packets.

Performs patient outreach to schedule patients for the Aorta Center.

Interact with co-workers, visitors, and other staff consistent with the core values of the University.

OTHER FUNCTIONS AND COMPETENCIES in addition to Essential Functions:

Provides Monthly Updates regarding volume, patient outreach etc.
