RN Registered Nurse Home Health -Contractor $120 Per Visit

Home Health Care Agency is seeking part time Registered Nurses with/without Home Health experience.

Patient service area is Portland/surrounding areas.

Apply your home health nursing expertise and provide high-quality care to our patients. We utilize the latest technology to provide an efficient complete plan of care, with user-friendly web-based documentation.

Responsibilities and Duties:

Perform evaluation tasks, including vital signs and medication review and complete SOC

Develops a Plan of Care based on the patient diagnosis and needs.

Dress or redress open wounds and assess progress of healing

Educate patients and families on home health care strategies and procedures

Make recommendations for devices or tools that might improve the patient’s quality of life

Coordinate with Occupational Therapists, Physical Therapists, Physicians and others involved in the patient’s care plan.

Participates in Case Conference, in-service and meetings

Coordinates and establishes programs geared towards patient care

Maintains all communication amongst field staff and office staff

Qualifications and Skills:

Registered Nurse in the State of OR

Home Health Experience is a Plus

Excellent communication and time-management skills

Compassionate and friendly demeanor

Ability to travel (locally) with reliable transportation
