Job Title: Care Advocate

Location: Office is located in Medford, but the position is overseeing the Josephine County area, must be willing to travel

Position Overview:

We are seeking a dedicated and experienced Care Advocate to oversee client care as well as support and manage a team of caregivers, ensuring they provide high-quality care to our clients in the Josephine County area. The Care Advocate will play a critical role in training, mentoring, and supervising caregivers, ensuring they meet both client needs and company policies. This position requires a strong leader capable of motivating staff, resolving challenges, and building positive relationships with both clients and caregivers to deliver exceptional service. This position requires attention to detail to ensure assessments and documentation for clients and caregivers is filled out in accordance with state law.

Key Responsibilities:

Caregiver Mentorship & Supervision: Visit caregivers weekly in client homes to monitor performance, offer coaching, and provide feedback to ensure effective caregiving.

Training & Development: Provide comprehensive training to caregivers to meet company standards. Conduct evaluations for all caregivers.

Caregiver Support: Serve as a point of contact for caregivers, offering guidance on caregiving techniques, policies, and procedures. Provide assistance in overcoming challenges and concerns.

Address Performance Issues: Manage performance-related concerns by implementing corrective actions and disciplinary measures when necessary.

Client & Caregiver Relations: Build and maintain strong, professional relationships with both clients and caregivers. Conduct client assessments, update families and caseworkers, and ensure that all client care needs are met.

Documentation & Reporting: Ensure proper documentation for client care is maintained in WellSky, OneDrive, and other relevant systems. Monitor and update medication administration records (MARs).

Collaboration with Healthcare Team: Coordinate with nurses and medical professionals to support client care, including medication management and other health-related needs.

Administrative Support updating service plans, caregiver profiles, audits

Ongoing Training & Support: Participate in weekly Care Advocate meetings, monthly caregiver safety meetings, and assist with training during caregiver team meetings.

Must be willing to care give when needed.

Minimum Qualifications:

Experience: A minimum of 2 years of caregiving experience, with a demonstrated ability to train and mentor other caregivers.

Supervisory Experience: At least 1 year in a supervisory role within a caregiving or healthcare environment.

Skills: Proficiency in Microsoft Excel and Word, with excellent data entry and record-keeping abilities.

Communication: Strong verbal and written communication skills with experience leading teams and training groups.

Organization: Exceptional organizational skills with the ability to manage multiple tasks and maintain accurate records.

Required Skills & Attributes:

Strong interpersonal skills, with the ability to establish rapport and respect with both clients and staff.

Problem-solving abilities and foresight to address issues proactively.

Ability to enforce company policies and procedures effectively.

Flexibility to take on various tasks as needed.

Capacity to work both independently and collaboratively as part of a team.

Must have a reliable vehicle, current driver's license and insurance.

This is a position that requires strong time management skills and organization. This is a position that requires traveling from client home to client home, must be good at multitasking and completing documentation on a laptop.
