As a Care Coordinator, you will work with a diverse team of professionals to manage and coordinate care and promote the health of individuals enrolled in Medicaid. This reduces unnecessary hospitalizations, decreases emergency room visits, and improves health outcomes.Who we are looking for:We are looking for compassionate and engaging individuals who work well on a team and possess a combination of communication skills, computer skills and a knowledge of community resources and services. You should be familiar with and sensitive to the specialized needs of people living with HIV, chemical dependency and mental health issues, people of color, homeless individuals, and gay, lesbian and transgender individuals. This position also requires clear documentation, time management, commitment, and self-motivation.Successful candidates will be able to build trusting, professional relationships with clients, co-workers and external service providers. If you have strong interpersonal skills and the desire to make a difference, theAlliance for Positive Healthwould like to talk to you!Job Duties:Conduct intakes, comprehensive assessments, and reassessments in order to identify health care, mental health, chemical dependency and social service needs.Develop care plans in collaboration with an interdisciplinary team of external and internal providers. Conduct case conferences on a regular basis.Refer clients to services including but not limited to medical care, mental health and substance abuse treatment, entitlements and housing. Advocate for and assist clients in accessing and maintaining these services. Provide crisis intervention and updates on the client’s crisis situation to the interdisciplinary team. Conduct home and field visits, as needed. Escort clients to appointments as needed to ensure adherence to treatment.Qualifications:Bachelor's Degree in Social Work, Human Services or related discipline with one yearexperience OR CASAC with two years' experience working with persons with chronic medical conditions, HIV/AIDS, mental illness, homelessness, chemical dependency, and/or other populations of persons in need.Experience working with electronic medical records and/or health information exchanges (e.g. HIXNY) preferred.Billingual in Spanish preferred.Must have a valid, unrestricted driver's license and own vehicle for frequent travel.Other Details:Non-exempt;full-time 37.5 hours per week. NO weekends, holidays or evenings.
