We’re looking for a Full-Time Case Management Assistants to join our Bronx-based team and provide direct support to individuals in need

Minimum Qualifications:

-Bachelor’s degree in Social Work, Psychology, Human Services, or a related field (Required)

-Entry-level applicants welcomed – but degree must be relevant

-Strong communication, organization, and interpersonal skills

-Passion for serving diverse communities in a healthcare or human services setting

-Bilingual (English/Spanish) is required!

Key Responsibilities:

-Assist patients with case management services and support

-Draft letters for public assistance, housing, IRS, schools, and more

-Connect patients to local and community-based resources

-Advocate for patients to secure necessary services and specialized support

-Collaborate with departmental coordinators and team members to ensure quality care
