Private Duty Nurse needed - Must have Medicaid Number (NPI)

We are seeking an experienced, compassionate LPN for part time overnights - 2 shifts per week, flexible weeknights.

Location: East Northport, NY

Rate: LPN, medically fragile rate

Schedule: Overnight, 2 nights per week

Home Environment: Warm, supportive, dog friendly family.

Case - Our son is 21 years old, G-Tube, non verbal and non mobile, must be able to lift 140lb from bed to chair and chair lift for morning school.

IMPORTANT: Must be an active NY Medicaid provider and have an NPI number

Requirements:

Active NY LPN license

Must be enrolled as a Medicaid provider

Valid NPI number required

Experience with Tube feeding and Tube care

Proficient with suctioning, airway management

Reliability and professionalism are essential

Home-care experience preferred

Patient Care Responsibilities:

Safety oversight in a medically fragile environment
