Assists with training class and in service education to certified home health aides and personal care aides. Effectively manages patients on tele-monitors. Demonstrates exceptional triage skills and appropriate judgement/assessment skills to be able to make independent clinical decisions in monitoring patients and conducts appropriate follow up as needed. Provides leadership by guiding, teaching, developing and motivating others to perform in a manner that reflects the values of the system. Documents timely and in accordance with agency policies and regulatory requirements. Communicates effectively and maintains effective working relationships with others. Assumes duties and responsibilities as assigned by the Clinical Manager or designee. Provides nursing on-call coverage for the Agency. Responsibilities:EDUCATION
Graduate from an accredited school of nursing, college or university
Bachelor's Degree (BSN) preferred
Current New York State RN license
EXPERIENCE
Minimum of two (2) years professional nursing experience
Experience in Home Health Care preferred
Supervisory experience preferred
KNOWLEDGE, SKILL AND ABILITY
Proficient clinical and assessment skills
Evaluate skill competency of home health aides and personal care aides
Must be able to relate clearly and effectively in a professional manner
Ability to display tact and sensitivity while maintaining confidentiality
Excellent written and verbal communication skills
Strong organizational skills and leadership ability
Computer knowledge, able to use Microsoft Office
Ability to supervise, provide direction and motivate workforce
Ability to exercise independent judgment in executing responsibilities
