Breaking Ground's programs and services help people experiencing street homelessness - especially those who have been on the streets the longest - to come indoors. Through our Street to Home outreach program and transitional housing resources, we help people get, and stay, on the path to a permanent home.The Clinical Coordinator, Light Touch Services provides supervisory oversight to a new and growing team of Housing Retention Specialists providing time-limited services focused on housing retention, resource development, and community integration using a developing Critical Time Intervention (CTI)-informed model to individuals and families exiting shelter and moving into permanent housing in several new developments in Brooklyn and Queens. The Clinical Coordinator is responsible for ensuring that service provision is of the highest quality and is consistent with CTI-informed principles in the developing model. The Clinical Coordinator oversees a staff of 2-4 Housing Retention Specialists and, depending on program need, may carry a small caseload of more complex clients. The position also liaises with community stakeholders, funders, property management, and real estate development companies, and serves as a member of the Light Touch Services’ management team. Provide clinical direction and leadership to the program; help troubleshoot, problem solve, and strategize solutions around client engagement and challenging behaviors
Manage caseload assignments and conduct case reviews with staff
Provide training to staff on Critical Time Intervention model
Oversee development and implementation of individual service plans; ensure documentation of client contact and progress is comprehensive and timely
Assess and evaluate client functioning
Supervise and conduct case conferences
Coordinate delivery of care with multiple service providers
Work collaboratively with property management teams as needed to ensure clients succeed in housing and avoid return to homelessness
Provide support and guidance to front line staff in managing difficult situations and successfully de-escalating conflict
Produce timely outcomes reports based on services provided
Perform other related duties as assigned
MINIMUM QUALIFICATIONS:
LMSW, LCSW, LMFT, LMHC or similar credential strongly preferred
Minimum 5 years of experience working with populations who have experienced or are at risk of homelessness.
Housing-based case management experience strongly preferred.
Previous supervisory experience and strong clinical and assessment skills.
Familiarity with Critical Time Intervention principles preferred
Proficiency in motivational interviewing techniques preferred.
Superb collaborative skills, ability to work successfully with a wide range of internal and external stakeholders.
Creativity, strong problem-solving skills
Demonstrated success in working in a fast-paced environment
Ability to communicate (verbally and written) with diverse populations and stakeholders
Ability to travel throughout the 5 boroughs via public transportation or vehicle
Strongly preferred: Valid NY Driver’s license with a driving record in good standing and maintaining a good driving record (as defined by the organization)
Preferred: Bilingual Spanish/English
