DD Waiver service coordinator

We are looking to hire an additional service coordinator, experience preferred but not required.

Must have experience working with people with disability

be compassionate, understanding

must be able to carry a case load of 18

conduct monthly home visits

maintain caseload files

Develop semi- annual reports

View family home book for compliance

work closely with director and trainers and agency nurse.

complete required training per DD waiver standards

Have reliable transportation

Current driver's License

Vehicle insurance/ registration

High School Diploma

Pass a background check

must have contact information with voice mail set up to leave messages,
