Home Health & Hospice Registered Nurse (RN), On Call Nights, Full Time,

As a full-service home care and hospice agency providing a broad range of services to our customers, we are currently seeking an experienced, full-time Home Health & Hospice Registered Nurse (RN) who will be on call every other week from 5pm to 8am. Evaluate and treat patients using the most current technology and practices

Develop starts of care based on communication with the physician and patient assessment

Participate in clinical outcomes monitoring, follow up, and agency performance improvement initiatives

Collaborate with an interdisciplinary team to provide highest level of care

RequirementsQualifications & Requirements:

Current RN license in the state of New Mexico

Minimum of one (1) year of hospice nursing experience

Current CPR certification

Reliable transportation, valid driver's license with no DUI's, revocations, or suspensions in the last 5 years

Exceptional customer care

Excellent written and verbal communication skills

MS Office skills (Word, Excel, PowerPoint)
