Position Overview:

The Housing Outreach Specialist is an independent contractor responsible for identifying and engaging individuals and families who need support with finding or maintaining stable housing. Through proactive outreach in community settings, this role focuses on referring clients who are 18 years or older and have a disability, including developmental disabilities, learning disabilities, mental health conditions, illnesses, injuries, substance use disorders, or other incapacities. The Housing Outreach Specialist will connect these clients to housing resources, such as rental assistance, emergency housing, and case management services.

Key Responsibilities:

Conduct outreach to identify individuals in need of housing support, specifically those facing homelessness or housing instability due to a disability.

Assess clients’ needs and refer them to appropriate housing programs and services.

Assist clients in navigating housing applications and securing long-term housing solutions.

Build and maintain relationships with local service providers, ensuring effective referrals and support.

Keep accurate client records and track the progress of housing referrals.

Qualifications:

Experience in outreach, housing services, social work, or related fields preferred.

Knowledge of local housing resources and support services.

Strong communication skills and the ability to engage diverse, vulnerable populations.

Ability to work independently and meet outreach goals.

Compensation:

For each individual you refer to housing services, you’ll earn a $100 referral fee. The person must have Medical Assistance (MA) insurance through UCare, United Health, Hennepin Health, or Medicare. Help connect clients to housing resources while earning compensation.
