Position: Human Services Program Coordinator Reports to: Human Services Site Manager Hours: Full-time/Salaried , M-F, 8am-4:30pm Location: Broadway Tria ngle’s Expectation of Employee:

Essential Duties and Responsibilities: 

Supervises staff of Community Coordinators (Direct Support Workers) and leads the delivery of community-based employment and activities aligned with Community Based Day/Employment Supports models

Assists with the training and orientation of new staff for software systems, programming components, and program related logistics

Coaches staff to provide individualized support and problem solving for participants and their families

Provides monthly, one-on-one documented supervision with Community Coordinators; enforces attendance and progressive disciplinary procedures as necessary

Ensures and maintains proper staff to participant ratios; provides direct care as needed

Along with Site Manager, leads hiring process for Community Coordinators (Direct Support Workers)

Coordinates with Site Manager and staff to create and implement monthly schedules and employment schedules for program; solicits and provides feedback on a consistent basis to improve quality of services

Supervises groups of participants in a variety of settings including work sites, volunteer sites, recreational outings, and other community outings

Coaches Community Coordinators (direct care staff) on work sites, assessments, billing, etc.

Maintains and builds professional relationships with companies with supported employment sites

Stays abreast on current utilization of employment contracts and adjusts weekly schedules as needed

Carts out appropriate units via Penelope on a daily basis , and adds service notes

Networks and connects with community organizations to set up opportunities including volunteering/internships, employer tours, social/recreational, employment skills and independent living activities, all performed in the community

Assists with the development of new and engaging curriculum to meet program goals

Transports individuals to and from community activities/employment sites

Prepares or assists in the preparation of required assessments and reports (i.e., incident reports, safety assessments, progress summaries, etc.)

Intervenes in any unsafe interactions or behavioral incidents that occur to ensure individuals safety

Collaborates with staff and Site Manager to ensure quality service delivery

Provides regular checks-ins with staff and program participants to ensure services relate to individual’s goals, needs, desires & interests

Ensures 50% of programming occurs in the community

Positively represents Triangle to customers, family members, and community organizations

Other duties as assigned

Primary Case Management Responsibilities:

Manages caseload of DDS, MRC, and private pay clients

Consults with Site Manager and staff to ensure consistent case management practices are followed

Collaborates with Site Manager and staff to write and submit appropriate ISP goals and to accurately track progress/identify challenges

Participates in ISP meetings with Department of Developmental Services, participants, and their families or other members of their support team as needed

Ensures 98% of ISP documents are submitted on-time and 80% of ISP goals are met

Monitors participant attendance and proper billing of services in Penelope database daily

Participates in agency wide committees and initiatives as assigned

Attends monthly case management meetings as requested

Takes direction from Site Manager to collect accurate data as needed for ISP’s, behavior plans, monthly progress notes, etc.; trains staff as appropriate

Checks and ensures completion of ISP, behavior, and other data; follows up as appropriate

Serves as backup quality check for monthly billing, client payroll, and other appropriate information

Other duties as assigned

Other Responsibilities:

Meets monthly with Site Manager for supervision

Participates in biweekly team meetings

Participates in identified training and professional development opportunities

Assists or prepares documentation for Human Rights Committee review as needed

Knows and adheres to all safety policies and emergency procedures

Assists with or leads intake process for new participants and completes all required consents/assessments in Penelope database

Completes monthly progress notes and responds to alerts in the Penelope database

Prepares and submits Incident Reports and ISP documentation via HCSIS as needed, and notifies Site Manager and other appropriate parties

Proactively communicates with families and funding sources

Reports progress and issues to Site Manager on a regular basis

Guides staff through accidents related to toileting, physical or medical needs as needed

Participates in 1:1 meeting with individuals on caseload as needed

Acts as a liaison to residential staff, families, therapists, and funding agencies to ensure continuity of supports

Participates in APLT meetings and Safety Committee; updates team as necessary

Other duties as assigned

Qualifications

High school diploma or equivalent

Bachelor’s Degree in related field preferred; may substitute with combination of prior experience/credentials

Direct care experience required

One to two years in a supervisory role and/or case management preferred

Willingness to cold call companies/businesses/organizations to create volunteer and employment opportunities

State/Federal background check

Massachusetts Driver’s License and clean driving record

Language Skills

Ability to read, analyze, and interpret documents. Ability to respond to common inquiries or complaints from regulatory agencies, or family members.
