Client Care Coordinator – Job Description

The following duties and responsibilities reflect the essential functions for this job, but do not restrict the tasks that may be assigned. The Executive Director may assign or reassign duties and responsibilities to this job at any time due to reasonable accommodation or other reasons.

Essential Functions/Tasks:

Schedules client cases with appropriate field of employees, making follow through calls necessary to produce an effective, smooth flowing schedule

Monitors vacation/time off requests submitted in writing from staff to ensure proper coverage is always in place for clients

Contacts clients regarding Holiday Coverage to determine needs and to remind of Home With You Holiday Pay Policy

Supervise the work of all agency non-medical staff and ensures the client’s Plan of Care is being followed

Coordinate caregiver/client introductions – if a new client, meet with the caregiver and client to make the initial introduction

Conducts annual employee performance evaluations in conjunction with the Executive Director

Investigates problems, irregularities, policy violations and other reportable events implementing the appropriate corrective action and follow up

Conduct New Hire Orientation

Participate in the referral/intake process for prospective clients needing home care

Works closely with office staff to promote inter-agency and intra-agency communication to assure appropriate client care

Participates in after hours and weekend on-call schedule

Maintain client charts in Clear Care to ensure compliance and accuracy

Conduct client visits as assigned at Monthly Staff Meetings

Assist potential applicants with application and conduct interviews in conjunction with the Administrative Assistant

Attend appropriate office meetings

Fill in for caregivers in emergency situations

Resolve client complaints/concerns as applicable and documents clearly and concisely

Performs other administrative tasks as assigned
