Strategizes methods to scale communities and drive greater connections among community members. Engages new community members.

Understands own work area professional concepts/standards, regulations, strategies and operating standards.

Work independently with with minimal supervision

Work is managed/guided by documented procedures/regulations/professional standards with some interpretation

Required Qualifications2 or more years experience with Case Management and/or Social Services job experience

Travel up to 60% of the week to meet members in their homes and/or in the community and must be willing to meet with Community Based Organizations on regular basis.

Intermediate working knowledge using Microsoft Office Word and Excel

Exceptional verbal/written communication and interpersonal skills

Ability to use a variety of electronic information applications/software programs

Tuberculosis (TB) screening program- This role is considered patient facing and is part of Humana's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.

Driving / Auto Insurance / Reliable Transportation- This role is part of Humana's Driver safety program and therefore requires an individual to have a valid state driver's license and proof of personal vehicle liability insurance with at least 100,000/300,000/100,000 limits.

Humana and its subsidiaries require vaccinated associates who work outside of their home to submit proof of vaccination, including COVID-19 boosters. Associates who remain unvaccinated must either undergo weekly negative COVID testing OR wear a mask at all times while in a Humana facility or while working in the field.

Preferred QualificationsBachelor's Degree

Home Health experience

Community Health Worker Certification

Experience with Medicare & Medicaid recipients

Previous experience with electronic case note documentation and experienced with documenting in multiple computer applications/systems

Experience with health promotion, coaching and wellness

Knowledge of community health, social service agencies and/or additional community resources

Bilingual English and Spanish

Location: Frankfort, KY surrounding area.

Hours: Must be available to work business hours Monday - Friday 8AM-5PM (EST), opportunity to move to 4 X 10's

Travel: Must be willing to travel up to 60% of the week to meet members in their homes and/or in the community and must be willing to meet with Community Based Organizations on regular basis.
