Coordinate and lead the development and implementation of the Community Health WorkerModel to continue and establish key partnerships within minority and marginalized populationsfor the Office of Minority Health. Supervise and provide support to the 10 regional community health workers. Devote 100% of time to development of operational procedures, programmatic logistics andadministration, supervision of regional CHWs, implementation of the dissemination plan and willserve as the primary liaison between the OMH, the Regional CHWs, Indiana Community HealthWorkers Association (INCHWA), Minority Health Partners, and Purdue University. Supports regional CHWs as they build and maintain strong cooperation with the local healthdepartments and established health councils or regional networks to keep them informed andactively involved in community health activities. Build and ensure strong partnership with state and regional partners.Principal Tasks: Assist with preparation of contracts and reports. Assist with data entry and data quality assurance processes. Processes claims justifications and invoices. Ensures notices from federal and state partners are communicated to CHWs and their regions. Support regional CHWs in strategizing to reduce barriers to health care access for underservedand marginalized populations Collaborate with INCHWA in providing technical assistance to the CHWs and their region. Represent the agency and the OMH in meetings with program partners. Support agency and division health equity efforts. Assist with various projects including but not limited to data-related projects with Health EquityCouncil, Health Equity Research Analyst, Health Disparities Epidemiologist, IHIE and projectswith the Office of Minority Health. Support agency COVID-19 pandemic response efforts as needed. Perform additional related duties as assigned.Job Requirements: A minimum of 5+ years of experience working closely with community health workers. Education: A master's degree in a relevant field (e.g., public health, biological sciences, or socialsciences), OR A doctoral-level degree in public health or another relevant field, OR A bachelor's degree in a relevant field and 2 years of relevant public health experience. Experience with working with communities, community-based organizations, and/or localcoalitions. Experience with programmatic coordination and planning. Experience with supervision and support.Knowledge, skills, and abilities: Ability to identify, communicate and work effectively with local health department staff,community organizations, members of the public, and medical providers. Working knowledge of Indiana's public health landscape including health inequities and socialdeterminants of health. Familiarity with communicating with diverse groups of people and the importance of culturallyand linguistically appropriate services. Ability to prioritize tasks efficiently to meet internal and external deadlines. Ability to appropriately document work activities and maintain organized records. Self-motivated, friendly, and professional. Strong oral and written communication skills. Strong organizational and time-management skills with a strong attention to detail. Ability to work efficiently and independently with minimal supervision. Ability to provide prompt, courteous, and helpful customer service. Strong critical and analytical thinking skills.
