Bilingual Spanish Public Benefits Specialist
Illinois, Chicago, 60601 Chicago USAWe are seeking a dedicated individual to join our team as a Case Manager/Public Benefit Specialist. In this role, you will provide comprehensive case management services to older adults, groups, individuals, and their families/friends who advocate for the client's needs. Working collaboratively with community resource partners, you will be responsible for linking clients to essential services to enhance their well-being and quality of life.
Key Responsibilities:
Client Needs Identification and Assistance:
Conduct thorough assessments to identify clients' unique needs, ensuring a holistic understanding of their circumstances.
Work closely with clients to select appropriate healthcare such as Medicare or Medicaid, based on their individual needs.
Emotional support by addressing the emotional well-being of older adults by using an assessment tool that looks for signs of depression, anxiety, and social isolation.
Public Benefit Enrollment Assistance:
Guide and provide support in navigating clients through the application process for various low-income programs, including but not limited to Extra Help, Benefits Access Application, Medicaid, SNAP, Medicare Savings Program, and utility assistance to ensure clients receive the benefits they are entitled to
Translation and Advocacy:
Offer translation services to facilitate effective communication between clients and service providers.
Advocate on behalf of clients to ensure their voices are heard and their rights are protected.
Translation of flyers, and other materials.
Telephone and Social Support:
Provide ongoing telephone support to clients, addressing inquiries, and concerns, and providing reassurance as needed.
Offer social support to clients, fostering a sense of community and connection.
Community Engagement:
Represent the organization at community outreaches and enrollment events, disseminating information about available services and resources.
Conduct informative presentations at various locations to educate the community about our available resources.
Collaborate with community partners to enhance service delivery and maximize support for clients.
Reporting:
Responsible for tracking daily client interactions, including call logs, application submissions, and demographic information using spreadsheets for accurate reporting.
Qualifications:
A bachelor’s degree in social work, human services, or a related field is preferred, as well as 3+ years of experience with public benefits enrollment with the older adult population.
Previous experience in case management, public benefit enrollment, or advocacy is highly desirable.
Strong communication and interpersonal skills, with the ability to interact effectively with diverse populations.
Familiarity with community resources and support services for older adults.
Ability to work independently and as a member of a team.
Respect for social justice and equity in service delivery.
Requirements:
Strong punctuality ensuring timely attendance and adherence to scheduled commitments.
Must possess the ability to collaborate effectively within a small, diverse team environment.
Must be able to read, write, and speak Spanish and English
Proficiency in computer skills beyond a basic level.
Reliable transportation including valid driver’s license and auto insurance coverage.
Ability to pass background and drug tests.
SPECIAL REQUIREMENTS AND PHYSICAL DEMANDS:
Requires safely lifting or moving up to 25 lbs. occasionally, for example, movement of materials, files, or supplies.
Regular requirement to stand, and sit for long periods, with some walking, occasional bending, stooping, squatting, pushing, and pulling.
The noise level is usually quiet to moderate.
