CORE JOB FUNCTIONS Assesses assigned patients and evaluates plans to include documentation of nursing care. Reports symptoms and changes in patients’ condition and vital signs. Modifies patient treatment plans as indicated by patients’ responses, conditions and physician orders. Reviews, evaluates and reports diagnostic tests to assess patient’s condition. Consults with physicians and other healthcare professionals related to assigned patients to assess, plan, implement and evaluate patient care plans. Prepares patients for, and assists with examinations, procedures and treatments. Considers patient age and culture during patient treatments and provides any needed information regarding treatment plan. Nurtures a compassionate environment by providing psychological support. Performs appropriate patient tests and safely administers medications within the scope of practice. Administers and maintains accurate records related to medications and treatments as per regulatory bodies, policies, procedures and physician orders. Communicates plan of care in a timely manner to patient and family, as well as the appropriate team members, ensuring compliance with all regulatory guidelines (i.e. HIPAA). Uses best practices for transition of patient care. Uses available resources to assist in discharge planning. Plans, prioritizes, and adjusts assignments to accomplish goals and render superior patient care; seeks assistance when needed. Adapts to changing work demands and environment. Safely operates medical equipment. Serves as a preceptor and assists new staff in the provision of care in order to help them acclimate to the healthcare environment, and a direct patient care role. Provides concise and constructive feedback when needed. Knowledge of medical terminology

Department Specific FunctionsMeets requirements of Pre/PACU Nurse Performs all responsibilities/duties required by the Recovery department/patient care services as specific tothe scope of service, to ensure that the unique nature of the patient is addressed.Demonstrates the ability to titrate intravenous medications according to the patient’s hemodynamic status. Initiates IV therapy for both OR/IR/GI Areas. Demonstrates the ability to care for the patient with continuous cardiac monitoring including rhythm identification, interpretation and follow through with appropriate intervention. Demonstrates knowledge of anesthetic agents, techniques and actions. Demonstrates the ability to care for the population served: post-op patient Demonstrates the ability to perform rapid assessment differentiating between short-and long-term needs. Verification of the operative procedure, operative site, surgeon and consents. Assesses psychological status, including level of understanding, perception of surgery/procedure, and anxiety level. Verification of patient allergies, NPO status, and whether or not patient has voided. Checks the current history and physical, laboratory results, the execution of Perioperative orders, and results of ordered diagnostic tests. Communicates relevant findings to Physician/Anesthesiology/CRNA/PA Assists and expedites the transfer of patients to and from the Recovery to In-Patient and or other facility Assists with emergency situations, such as rapid response and Code. Functions in the assessment and care of both pre/post post procedure monitoring. Communicates to accepting staff via Hands-Off Communication. Ensures patients’ privacy, comfort level and complaints are responded to immediately. Assists with pre-procedure calls to patients, as well as post-procedure calls (PCM). Engages and is committed to UHEALTH Service Pillars, AIDET and telephone etiquette. Follows UHEALTH Standards of Behavior.Assessment Performs assessments, including vital signs and reassessments focusing on the physical and psychosocial health status of the patient in an ongoing timely and systematic manner in both the pre and post areas and enters in the Electronic Medical Record. Obtains baseline information utilized for the development of individualized patient plan of care. Provides direct and indirect patient and family care to assess needs. Facilitates/patient/family education with interdisciplinary team involvement. Monitors patient’s progress against plan of care until discharge from designated patient care area. Collaborates with interdisciplinary team members to identify potential barriers and variances to the plan of carePlanningCoordinates the plan of care with patients, family and interdisciplinary team. Ensures the plan of care is accessible to the patient and family to assist them in understanding goals of care and movement toward these goals. Plans discharge and teaching needs as applicable. Ensures patients and family fully understand discharge instructions. Promotes development of clinical practice based on research and evidence. Assists leadership team with maintenance of initial and ongoing unit-based clinical competenciesImplementationActs as a liaison between patients, caregivers and community resources. Educates interdisciplinary team, patient/family on plan of care. Serves as a direct care provider, as appropriate, to ensure outcome obtainment. Serves as preceptor to new staff. Documents concise, accurate and legible according to nursing policies and procedures and nursing standards. Provides patient/family teaching using available resources and tools. Acts as consultant to clinical staff in assisting with unfamiliar procedures/treatments. Utilizes process of performance improvement. Promotes patient safety. Communicates changes in plan of care to the patient/family and the health care team members in a timely manner. Demonstrates creativity and innovation in approach to complex problem solving. Leads crisis management. Delegates’ aspects of care to qualified personnel. Administers and/or monitors blood and blood products following hospital policy and procedures Demonstrates the skills and judgment necessary to implement medical plan of care, nursing intervention and procedures as necessary for the care of the patient Delivers sedation and analgesia according to hospital policy and procedure. Demonstrates knowledge of commonly used meds: action, dose, and side effects. Observes five Rights for administering meds: right patient, right drug, right dose, right method, and right time Medication Administration as per hospital policy and procedureEvaluationEvaluates patient for improvement after implementation of plan of care to assess for the attainment of goals. Readjusts plan of care based on evaluation of the patients’/families’ needs. Monitors outcomes of patients undergoing new procedures or interventions. Evaluates staff competencies and reports to leadership team. CCRN or CPAN preferred
1-2 years recent Recovery, ICU, ER, or Telemetry nursing experience preferred
Strong interpersonal communication/customer service skills
Moderate Sedation Competencies as appropriate
BLS, ACLS, PALS
