What you will do:Visiting hospitals, physician offices and other healthcare facilities

Records new referrals and updates record status changes daily

Prioritizes referrals, discusses with team and physician as needed each day.

Identify referrals that require consult orders and/or therapy orders and obtain orders within 24 hours or less.

Notifies referral source regarding status of referral ; providing education, encouraging future referrals, creating the WOW experience during every interaction.

Records consults and updates record status changes daily

Identify consults that require additional information or therapy orders and obtain orders/information within 24 hours or less

Prioritizes each physician  consults, discusses consults with each physician in the morning.

Reviews patient charts and physician consults; obtaining all necessary information to complete regulatory documentation and discuss case as needed with physician and team.

Establishes and maintains frequent positive communication with stakeholders in acute care and outside facilities re: the status of the patient  IRU admission

Communicates daily with the Pre-admission team regarding any consults which are 1.Greater than 3 days 2. Potential denials for any reason except insurance denials 3.Potential admissions that do not meet the full criteria and require team input.

Actively and positively communicates with Case Management and consulting physician regarding any patient that was denied explaining reasons for denial, encouraging additional referrals and answering any questions; creating the WOW experience during every interaction.

Completes requests for financial clearance on the same day as patient is approved by physician for admission.

Provides accurate and legible information regarding patient insurance carrier, policy numbers and authorization codes on the financial clearance request forms.

Gathers all pertinent information needed to obtain necessary authorizations preventing delays due to incomplete information.

Communicates with insurance companies; completing computer generated forms, fax etc to obtain authorizations/pre certifications as required to complete the financial clearance process.

Oversight of the pre-admission screening form and process following all regulatory requirements.

1. Reviews all forms for accuracy, timeliness and completeness.

2. Scans or enters information into the computer

3. Maintains backup copy of all handwritten forms

4. Notifies supervisor of any form which is not compliant

5. Notifies supervisor of any potential regulatory non-compliance issues

Actively participates in any meetings (ie. daily rounds) which help to identify patients that might benefit from IRU admission.

Responds positively to any inquiries regarding admission criteria on IRU from interested parties.

Provides education and information to all interested parties; including healthcare fairs, physician office visits, skilled nursing facilities visits, unit tours, which will elicit increased referrals and admissions to IRU.

Maintains supply of printed marketing information to include brochures, helpful patient information handouts and other related materials.

Assists in the marketing activities; including assisting with HCH web information and updating to promote services offered by the program.

Answers all telephone calls in a prompt, courteous and efficient manner. Treats all calls as being important.

Coordinates admission referrals and consults from external facilities to HCH using same process as internal referrals and consults.

Notifies and provides completed admission documentation to the hospital admissions office.

Communicates admission information within 30 minutes to the charge nurse, nurse manager(or designee) and IRU therapy scheduler.
