QUALIFICATIONS:
-Proven 2+ years experience as a medical practitioner (PA/DO/MD)
Excellent medical skills in diagnosis and treatment in primary care, acute care, palliative, EOL, and chronic care management
Has a familiarity with geriatric psychiatry, palliative and end-of-life care employing integrative modalities to care
Must hold current Maryland License and DEA, NPI. PECOS/Medicare credentials
Have own auto, own computer - 1099
Knowledge of Medicare rules and regulations
Outstanding charting skills in creating comprehensive SOAP notes (dictation system available)
RESPONSIBILITIES:
-Examine elderly home-bound patients and their medical records
-Diagnose, treat, and manage complex acute and chronic disease states
-Order in-home studies, diagnostic tests (labs, imaging, etc.)
-Maintain accurate records and schedules
-Educate patients and their family members on diseases, epidemics, and prevention
-Coordinate care with a team of professionals such as home health and hospice agencies. Work closely with allied healthcare professionals
-Maintain and keep current licenses and malpractice
-SEE YOUR PATIENTS' HOSPITALIZATIONS DROP AND QUALITY OF LIFE IMPROVE
