Would you like to become a Host Home Provider? Do you have a wheelchair-accessible home, or are you willing to modify your home to make it wheelchair-accessible? Do you believe that everyone, regardless of ability, deserves to live safely, happily and meaningfully within their community? Are you looking for a long-term relationship with an experienced company and a supportive culture? If so, we have a special program for you!

We are committed to developing long-term relationships between individuals and providers, so we only introduce people who seem to be a great long-term match for one another. We understand the commitment involved in being a host home provider and we know that serving someone who requires wheelchair accessibility involves a special commitment. For these reasons, we are also offering outcome-based payments for long-lasting partnerships. While PPCH offers comprehensive supports and competitive daily rates for all host home providers, for those who qualify, we are also offering an additional:

1. $500.00 – 90 days after you begin serving an individual

2. $500.00 – 180 days after you begin serving an individual

3. $1,000.00 – 1 year after you began serving an individual

How to Qualify:

1. This program is open to new providers only. Only applicants who have not served clients with PPCH previously can qualify.

2. Providers must have a home which meets applicable standards for wheelchair accessibility. If your home is not wheelchair-accessible, you may still qualify for this program if you modify your home to meet our standards. Please see the attached guidelines for more information about wheelchair accessibility.

3. You must serve an individual who needs a wheelchair accessible home for a qualifying period (90 days, 180 days, 1 year).

4. You must meet PPCH standards for compliance in the following areas: household/screening, home inspections, contractor file, training, service documentation, appointments, billing and attendance.

Providers must meet all criteria above in order to qualify for PPCH’s outcome-based payments.

In general, host homes must be wheelchair accessible prior to serving individuals who require wheelchair accessibility. A temporary exception may be granted if only smaller modifications remain and have been scheduled within a reasonable time; these can be discussed case-by-case.

For providers who do not make the qualifying period, but who otherwise honor their contracts, no penalties or fees apply, you simply will not qualify for the additional payments; standard reimbursement will remain unchanged.

Frequently Asked Questions:

What if I already applied to a different posting and want to be considered for this one? For those who have applied to other host home postings, we will ask questions to see if you qualify for this program and can provide you with more info if you do!

What if I already serve an individual, either with PPCH or with another company? Can I still qualify? This program was approved for new PPCH providers only, but we love partnerships! If you currently work with another PASA and are interested in serving another individual, we would love to hear from you! If you already serve an individual with PPCH, you do not qualify for this program.

What if I change my mind? There is no penalty if you or your team find a better match, please just remember that you will not qualify for the outcome-based payments. Please also be sensitive: if you change your mind after we have introduced you to an individual or family, this can have serious consequences for them and their services. It will be best if we all do our research ahead of time to find the best program for you and your home.

What about referral bonuses? Are they honored in conjunction with this program? Yes! This program is not intended to replace referral bonuses. It is intended to reward new providers for developing and maintaining wheelchair accessible supports and services.

We provide all of the necessary training for our Host Home Providers to be successful in the field including but in the areas of Developmental Disabilities, CPR, First Aid, medication administration and behavioral safety training, Rights, Confidentiality etc.
