Position: PT - Home HealthPosition Summary

Provides physical therapy services in clients’ homes as prescribed by the physician and offers consultation and education to agency staff.Level of Responsibility

Works under agency policies and client care plans. Reports to the Argus of Colorado administrator or designee and supervises assigned personnel.ResponsibilitiesPatient Care:Conduct initial and ongoing evaluations, including OASIS assessments.Develop and update care plans with the physician and care team.Identify barriers, set goals, provide treatment, education, and equipment.Coordinate services and plan discharge.Evaluate care effectiveness.Documentation:Record evaluations, treatments, and client responses promptly.Document conference outcomes and quality assurance data.Attend care conferences, staff meetings, and in-service programs.Provide education and consultation to clients, families, and staff.Supervise aides, therapy assistants, and paraprofessionals.Maintain professional knowledge through education, professional organizations and workshops.Perform other related duties as needed.
