California, Ventura county, 93001 Ventura county USAOVERALL OBJECTIVE: Provide all Social Work services for Ventura County Adult Day Health Care Center. All policies and procedures of Ventura County Adult Day Health Care Center, applicable laws and licensing requirements will be always followed.
RESPONSIBLE TO: Program Director
AREAS OF SOCIAL WORK RESPONSIBILITIES:
1. Process all inquiries in a timely manner and maintain tracking of documentation. Meets with family members, or a referring agency, and determines an appropriate program.
2. Provides in-home assessments for all ADHC participants. Complete Social services assessments on all participants.
3. Provides discharge planning for all participants.
4. Assesses and make recommendations for community resource needs of participants.
5. Provides full case coordination for ADHC participants and comprehensive information and referral resource file for center’s primary and secondary service area along with pertinent information for outside referrals of service area.
6. Provides signed and dated documentation for all services performed on the day the service is provided.
7. Completes signed and dated quarterly progress notes, and six-month reassessments on all participants.
8. Provides group sessions in the program.
9. Provides participants and family counseling as needed.
10. Maintains a working knowledge of the facility service areas including transportation, community characteristics and geography.
11. Actively participates in ADHC Multi-Disciplinary Team.
12. Actively participates in all staff and in-service meetings, requirements may require outside training. Provide training for the service as assigned.
13. Works as an integral participant in a management team concept. Maintains effective relationships with staff geared to the building and maintenance of cohesive team.
14. Consult with and advise staff members as to the relationship of social, emotional and ethical factors to health and medical care, and as to the availability of social services in the community.
15. Maintains all agencies, licensing, contracts and administrative requirements for documentation as required for clients’ services and administrative needs including raw data collections and tabulations and reporting requirements.
16. Assist in developing and providing educational training for caregivers and professionals. Assisting in providing fact sheets and other information materials.
17. Acts as an advocate for elderly concerns and provides referrals to people inappropriate for the adult day health center.
18. Participates as a member of assigned community committees as a representative of Ventura County Adult Day Health Care Center.
19. Maintains a networking liaison with other organizations in the eldercare field. Represents the agency in contact with human service, health care, and community organization groups and individuals.
20. Primary person responsible for providing marketing and public relations efforts for the facility. Plans will be established with administration including goals and tracking.

MINIMUM QUALIFICATIONS:
A master’s degree in social work from an accredited school of Social Work is required, with three years of recent case management experience and two years of geriatric experience is required. Another graduate degree may be considered with approval from the state of California. Must demonstrate effective oral and written materials and make presentations to community groups. Must possess effective time management skills. (Prefer Bilingual SPANICH).
CONDITIONS OF EMPLOYMENT:
Physical clearance for reaching, bending, stooping, crouching, kneeling and grasping; have full range of motion; ability to life 50lbs; may need to lift and transfer participants; T>B> clearance; must maintain current First Aid and CPR; Must have reliable private vehicle, proof of CA driver's license and maintain proof of current vehicle insurance
