Program Description:

Transitioning to adulthood is hard for anyone, but can be especially challenging for our most vulnerable current and former foster youth. TLC provides comprehensive mobile case management including employment, health and wellness, education, finance management, accessing resources, and navigating the ins-and-outs of independently living. We help youth (ages 16-25) to rise up from foster care successfully by providing a safe housing environment where they can learn and practice the life skills necessary to work on self-identified life goals.

Position Summary:

The primary functions of the Social Worker are to uncover barriers to meeting independent living skills and goals identified in the participant’s youth-centered assessment plan, coordinate services, link necessary resources, and support youth’s ongoing daily challenges of independent living. Social Workers have a caseload of no more than 15 youth and engage in weekly contact with program participants within the participant’s home and community. This position requires dedication, commitment and patience in developing effective case management practices and plans while implementing policies and procedures in a teamwork model and harm reduction approach in accordance with agency policy and California Community Care Licensing Regulations, Title 22, Division 6. This position requires flexibility in personal scheduling and meeting a variety of needs as assigned on a daily basis by the immediate supervisor. Rotating on call, holiday, night and weekend hours required. This employment opportunity requires the employee to live in Sonoma County.

Qualifications:Master’s degree in social work, or a related field preferred with at least one year working with transition aged youth (16-25 years) BA applicants welcomed with at least 2 years experience working with foster youth and 3 years of case management experienceMust be able to transport the youth in employee’s personal vehicle and have a clean driving record, valid CA driver’s license, and maintain minimum vehicle liability and property insurance limitsEssential Functions:Conduct assessments of participant’s strengths and needs and advocate for them accordinglyAid each youth as they develop and meet the goals established in their goals assessment plan, outlining personal, financial, educational, employment and housing objectives and carry out appropriate interventions according to case management practiceMaintain case records and provide timely reports on progress and outcomesOversee arrangements for transportation to appointmentsMonitor participant’s participation in employment, education, and independent living skills trainingEnsure participant’s compliance with program rules and responsibilitiesMaintain appropriate working relationships with private and public agencies for the purposes of effective case management and collaborationCompliance with all applicable California State Community Care Licensing regulationsProvide supervision of Transitions AdvocatesSkills:Demonstrate capacity to establish rapport and motivate youth and young adultsPassion for working with a diverse group of youth including LGBTQI, youth with disabilities, and youth from a variety of ethnic and cultural backgroundsUnderstanding of a youth-adult partnership frameworkHave an understanding on the effects of complex trauma and relationship to mental health and substance abuseA strength-based perspective is essentialStrong ability to organize and prioritize to balance documentation and youth needsTLC offers a generous benefit package for full time staff including:

Salary Range: $50,960 – $65,549 per year, depending on years of experience.
