Care Manager in West Marin

Join the team at West Marin Senior Services as a Care Manager.

Responsibilities include conducting in-person and/or phone interviews, and annual assessments. Develops, implements, and monitors care plans for clients. WMSS Care Managers work to build strong bonds and trust with ongoing evaluations to ensure client success.

LCSW or RN desirable, 4-year college degree, background in social work, nursing, or homecare required.

Bilingual skills in Spanish and English are highly desirable.

A valid driver's license and cell phone are required.

The service area includes San Geronimo Valley, Nicasio, Pt. Reyes Station and Inverness.

Hours: 32 per week, hybrid/remote work schedule may be possible after 90 days.

Salary: $30 per hour.
