Case Manager

IMT Associates, a private professional fiduciary practice, is seeking a case manager. Our practice focuses on the administration of trusts, estates, conservatorships and case management of elderly, mentally ill and disabled clients. This individual must enjoy working with our client population and do well in challenging situations. This person must also be dependable, honest, compassionate, professional, punctual, detail oriented, highly organized, and an excellent communicator. We are looking for someone who can balance both field work and office/administrative projects.

Primary Responsibilities:

Manage caseload of 10-15 clients.

Conduct client visits at home or in assisted living/skilled nursing facilities.

Engage with facility staff or in-home support teams to ensure client’s needs are met.

Assess clients’ housing situation and support necessary changes.

Keep key stakeholders (family, friends) informed of client’s status & changes, encourage social connections.

Understand client’s financial situation and manage to the established budget.

Handle various aspects of trust and estate administration duties.

Skills and Qualifications:

Use of own insured vehicle.

Available to work weekdays at our office in San Leandro.

Bachelor’s degree and minimum of 5 years’ experience in related field.

Proficiency in Microsoft Word, Excel, and Outlook.

Knowledge of social security benefits is a plus.

Knowledge of trust and estate administration is a plus.
