ABOUT THE POSITION:

As Care Manager for Hospice of Santa Barbara, your primary responsibility will be to facilitate the planning, implementation and monitoring of a plan of care for an assigned group of patients and/or families. As a key member of the Patient Care Services team, you will utilize your specialized training to provide compassionate care and support for those facing circumstances related to illness and/or end of life. This position is full-time at 40 hours per week with some remote work possible based on the needs of the agency and patients. Through your meaningful contribution, the Patient Care Services program will continue to blossom, answering the needs of our community. Specifically, your essential duties and responsibilities will be as follows:

Perform an initial, comprehensive psychosocial, spiritual, physical assessment for patients and/or families. Reassess periodically. Provide guidance, as needed, on entitlements/benefits or other financial or community resources.

Identify and integrate the psychosocial issues into goal-directed plans of care, working in collaboration with the Patient Care Services team.

Perform short-term crisis intervention and individual and/or family counseling as appropriate.

Collaborate with Care Coordinators, Director of Volunteer Services, Patient Care Volunteers and community partners to support the Patient Care Services program, including presenting in volunteer training modules.

Raise the community’s awareness about the emotional, social and spiritual healing opportunities death and grief present by performing a minimum of 4 community presentations annually; effectively communicate HSB’s unique services to medical and social services persons and agencies for the purposes of creating a pipeline of referrals for HSB’s Patient Care Services program.

What You’ll Need:

MSW, ASW, or LCSW

Demonstrated ability in care management and counseling, including familiarity with community resources

Care management experience preferably in either a hospice or a health care setting

Experience collaborating with volunteers

Knowledge of local (Santa Barbara) medical, health care agencies, and social service agencies

Experience in public speaking; effective presentation skills

Effective time management and prioritization skills

Ability to give and receive constructive criticism

Ability to reason, deduce and carry out complex multiple-step processes

Commitment to HSB’s vision and mission; passion for our cause

Ability to multi-task and effectively prioritize competing priorities

Excellent written and verbal communication skills; ability to communicate with all staff and members of the Board of Directors

Commitment to high ethical standards and professionalism as per our internal workplace standard

Must have own transportation

The salary range is $54,300 to $96,400 and is dependent on the level of qualification.
