Program Manager Adult Day Program

This position is responsible for supervising the day-to-day operations of the day program; develop and coordinate ISP’s/Person Centered Plans which would provide for the maximum growth potential of individuals with developmental or physical disabilities.
Duties/Responsibilities:
Plans and facilitates program activities that are tailored for individual participation and development.
Supports participants to best advocate for themselves and participate in informed decision making.
Writes, revises and updates the Individual Service Plans (ISP)/Person Centered Plans (PCP).
Writes, revises and updates goals & objectives for participants in alignment with their PCP.
Provides follow through on the individual’s person-centered plan to assure that they are properly implemented, monitored, documented and evaluated to meet program requirements.
Attends and conducts regularly scheduled trainings and staff meetings.
Develops and implements non-aversive behavioral management plans to meet the needs of the individual.
Operates personal or agency-owned vehicle in a safe manner and in accordance with CA State law.
Implements and trains staff in safe practices including hand washing, hand sanitizer, donning and maintaining face mask/or shield and general infection control and be the infection control lead.
Maintains a professional demeanor with employers/families/care providers and other professionals while in the community, and with all outside agencies.
Maintains Code of Safe Practices for his or her classification.
Adheres to the Rights of Persons with Developmental Disabilities, maintains participant confidentiality; follows required reporting procedures with respect to incidents or suspected abuse.
Monitors any medication changes and updates medication sheets as needed. (if no nurse)
Follows Title 17 & Title 22 regulations regarding day programs.
Submits required documentation per Title 17 & Title 22 regulations.
Notifies ACRC both verbally and in writing of any participant absences (5 days or more).
Assesses participants for entry in the day program and conducts tours of facility.
Ensures that the day-to-day operations are complete with assigned tasks and activities.
Responsible for making sure NovaTime is completed and accurate.
Responsible for monitoring and recording daily attendance.
Responsible for the acquisition of supplies, equipment and other materials within the programs budgetary limitations.
Maintains facility by contacting appropriate vendors for necessary repairs.
Ensures the facility is clean and working order.
Maintains licensing standards and program records, including monthly attendance, participant records, data collection and other forms necessary for program accountability and compliance with Title 22 & Title 17.
Recruits, hires, counsels and trains staff following UCP policies and procedures.
Provides daily supervision of staff, conducts annual evaluations.
Communicates and works with all UCP departments.
Other duties assigned.
Required Skills/Abilities:
Excellent verbal and written communication skills.
Proactive and independent with the ability to take initiative.
Excellent time management skills with a proven ability to meet deadlines and monthly/annual goals.
Has the ability to quickly learn new systems as they are implemented.
Education and Experience:
· Bachelor's degree or equivalent experience in related human service field.
· Two years previous experience working with the developmentally or physically disabled.
· Minimum of two years supervisory experience.
CPR and First Aid certification (can be provided by UCP).
Valid driver license and clean driving record, Insurance
