Position: Registered Nurse (RN) / Licensed Vocational Nurse (LVN)

Type: Full-Time / Part-Time / Per Diem

Location: Long Beach, Anaheim, Santa Ana, Costa Mesa, Inglewood, Los Angeles, Pasadena, Simi Valley, Bakersfield, Santa Clarita, West Covina, West Hollywood, Culver City, San Fernando, Winnetka, Tarzana, Encino, Sherman Oaks, etc

Responsibilities:

For Registered Nurses (RNs):

Conduct comprehensive patient assessments to develop and update individualized care plans.

Provide direct nursing care, including pain and symptom management.

Collaborate with interdisciplinary teams to coordinate care services and ensure the highest quality of care.

Educate patients, families, and caregivers about disease progression, medication administration, and care techniques.

Maintain accurate and timely documentation in compliance with regulatory and organizational standards.

Participate in on-call rotations to provide 24/7 support as needed.

For Licensed Vocational Nurses (LVNs):

Assist in implementing care plans as directed by the RN and hospice team.

Provide direct patient care, including administering medications and treatments as prescribed.

Monitor patient conditions and report changes to the RN or physician.

Offer emotional and practical support to patients and families.

Maintain detailed documentation of care provided.

Qualifications:

For RNs:

Current and valid RN license in California.

Minimum 1 year of nursing experience; hospice or palliative care experience preferred.

Excellent communication and organizational skills.

Ability to work independently and as part of a multidisciplinary team.

For LVNs:

Current and valid LVN license in California.

Minimum 1 year of nursing experience; hospice or palliative care experience preferred.

Strong interpersonal and clinical skills.

Reliable transportation and ability to travel to patient homes.

For Both Positions:

CPR certification (current).

Knowledge of hospice philosophy and care standards.
