Arizona, Phoenix, 85001 Phoenix USAThe Community Health Worker - Tribal Practices provides culturally relevant education on prevention and the promotion of holistic wellness for Indigenous people residing in Maricopa County. Additionally, this position is a liaison between the NATIVE HEALTH Community Health and Wellness Department, Medical Department, and community members to facilitate access to services to improve diabetes-related health outcomes. The CHW reports directly to the Indigenous Wellness Coordinator.  KEY ACCOUNTABLILIES: Provide Diabetes-Related Education Community Health Outreach CommunicationProgram Development NATIVE HEALTH Indigenous Wellness Camp Records Management Information Preparation Garden Space Patient Care Supports the organization as neededKNOWLEDGE, SKILLS, AND ABILITIESFacilitate group classes proficiently using a wide range of skills and techniques to help participants understand their common objectives and assist them to achieve them.Understand how social determinants of health impact the community and participants.Knowledge of Arizona Indigenous tribes' practices of framing, gardening, and traditional foods.Knowledge of client confidentially and client rights according to Health Insurance Portability and Accountability Act (HIPPA).Ability to pass blood pressure and blood glucose testing competencies.Strong problem-solving skills and assertive in challenging situations.Highly resourceful team-player, with ability to also be effective independently. Creative thinker, who actively seeks opportunities and proposes solutions.Demonstrates ability to write clear and concise reports of service activities and other programmatic reports.Strong interpersonal skills including tact and flexibility to work effectively with supervisor, staff, and participants.Advanced computer skills included MS Office, specifically EXCEL, PowerPoint, Outlook, and Word. Develop creative ideas, manage, and deliver diabetes-related education via digital content using a range of techniques, across channels including websites, social media, and Zoom platform.Culturally competent; able to work with diverse workforce and client popluaiton.
