Direct Support Provider

Exciting opportunity to work with clients with developmental disabilities.

This is a 40 hours per week flexible hours.

Duties include (but are not limited to):

Provide one-on-one non-medical care to a male client in home setting and in the community.

· Follow the client's Plan of Care which may include, but not limited to the items outlined below:

· Provide companionship, conversation, mental stimulation

· Provide transportation to community events and appointments

· Encourage, exercise, observe, document and report client's goals and objectives in both home setting and in the community. This position will become 40 hours per week. Must be available to work at least one day out of the weekend. Time varies depending on client's needs.

Requirements:

Attained High School Diploma or Equivalent (GED)

Credentials / licenses / certifications: None

Months of experience: 6 preferred but not required. Willing to Train

Driver's license: D-Private Vehicle class

Driver's license endorsements: None

Additional skills required:

An applicant must have or be able to obtain the following:

Must be at least 21 years of age.

High school diploma or GED.

Must be able to communicate both verbally and written.

Valid Driver's license.

Current car insurance

Reliable transportation

Current First Aid/CPR

Negative TB test done within the last year.

Must pass a federal background check.

Must have the Ability to provide skill training, monitoring, supervision and support as indicated in Plan of Care. Direct Support Providers- Day Habilitation Workers/In Home Supported/ Respite provide these services in partnership with families and other team members.

Has a strong written/oral communication skill with diverse populations including families, team members and the public.
