JOB REQUIREMENTS: The purpose of this position is to provide support to
the social services and activity department for the long term care
units, Crossroads and Pathways. This position is supervised by the Long
Term Care Social Services Coordinator. Essential Duties and
Responsibilities The following duties are normal for this
classification. These are not to be construed as exclusive or
all-inclusive. Other duties may be required and assigned. 1. Assists
Client Services in an efficient manner. Complete assigned paperwork,
collect and organize data, document in EHR, and make collateral contacts
to ensure compliance with DHS regulations. 2. Assists as needed with
client appointment coordination, state survey, questionnaires,
transportation and legal appointments, such as court hearings or court
mandated treatment. 3. Monitors client chart audits and completes
appropriate paperwork in timely manner. 4. Assists residents with
acclimating to the facility, and continuing assistance with
communication to the resident, guardian, family, or other responsible
party. 5. Works with clients regarding weekly budget & money
allocations. Notifies business office to work with guardian for large or
additional money requests. Completes weekly resident shopping. 6.
Schedules and coordinates care conferences according to the Minimum Data
Set (MDS) published calendar. 7. Interviews residents upon admission and
yearly for social history, trauma informed care, and other assessments.
8. Assists with preparation for psychiatrist rounds, which includes
preparing paperwork and behavior counts. 9. Performs other clerical
tasks as needed (sorting, calculating, collating, stapling,
prooing, faxing, etc.). 10. Responsible to know and practice the
safety policies of the County and Norwood. Perform all job tasks in a
safe and responsible manner. 11. Assists activity coordinator with unit
activity programming and outings. Additional Tasks and
Responsibilities 1. Communicates and interacts professionally,
effectively and tactfully with residents, patients, physicians,
visitors, families and peers. 2. Participates in in-service and other
training as assigned. Completes annual job specific trainings and core
competencies as required by Norwood Health Center and Wood County. 3.
Reserves meeting rooms as requested. 4. Other duties as assigned.
OTHER EXPERIENCE AND QUALIFICATIONS: Associate's degree in a
human services related field desired. Two years of related experience
required. Must be able to maintain a pleasant disposition at all times.
Knowledge of computer skills and data needed. Ability to work
independently. Able to demonstrate flexibility with assignments. Ability
to concentrate. Accuracy is essential in all aspects of job
responsibilities. Able to exercise judgement, especially where clientele
are involved. Excellent listening skills, written and oral communication
skills, including English usage, grammar, punctuation and style.
