The Anacortes Family Center (AFC) is seeking a dedicated and experienced Full-Time, Exempt, Case Manager to assist clients in our program with achieving self-sufficiency and success within 60-90 days. AFC provides transformational and intensive wrap-around services aimed and supporting the clients short-, medium- and long-term goals. We do this through providing emergency shelter, very frequent case management meetings (or case management check-ins), weekly life skill classes, and referrals to additional services including mental healthcare, chemical dependency support, parenting support, or a variety of other services and resources to support the family with the aim of guiding the household out of crisis and transitioning into self-sufficiency and permanent, safe housing.

Schedule: Thursday-Monday, 8AM-5PM (schedule flexibility is needed to conform to the client’s schedules)

Reports To: Program Director

Compensation:

Salary: $40,000-$45,000 Annually, DOE, Performance reviews occur each May with merit and cost of living raises discussed each year.

o When financially viable, the Board of Directors does consider an end-of-year bonus tied to merit and seniority.

Includes a VERY generous benefits package including:

o Vacation Leave: Accrued at 80 hours per year through the first 24 months of employment, hours per year

through the 60th month, and 160 hours per year thereafter,

o Sick Leave: Accrued at 96 hours per year (one work day per month)

o Retirement: Employer-matched IRA contribution of up to 3% of annual salary

o Self-Care: The organization has a formalized self-care program wherein an assigned staff counselor is available to meet by appointment at any time. We also hold regular in-services trainings, we have a dedicated budget for ongoing professional education, and each employee receives a $200/year stipend to pay for anything they deem self-care.

Job Description:

Anacortes Family Center's Case Manager assists up to nine (9) households at a time living in our Emergency Shelter facility. Through meeting with the client several times per week (perhaps 1 one-hour meeting and several 15-minute check-ins throughout the week), the Case Manager will create a highly individualized Case Management Plan that will address the clients’ needs, their goals, and their barriers that the Case Manager will then pair to resources and providers internally and externally to support the family. The overall goal of the Case Manager is to help the client move from homelessness to self-sufficiency and permanent housing through providing direct case management services. The Case Manager is responsible for working one-on-one with each household, building the relationship and ensuring that there is good progress made on the household’s weekly action plans; and that the household is meeting program requirements and taking steps to overcome barriers, and supporting the client to achieving their goals. The Case Manager serves as a link to community resources and service providers.

High Level Functions:

Consult with Community Resource Manager to provide assessment of possible clients for intake

Provide one-on-one case management services to individuals and families experiencing homelessness.

Meet with each household 1-3 times per week formally to update action plan, assess needs and provide referrals.

Collaborate and network with community and other social service agencies to become familiar with local programs and to provide appropriate client referrals.

Ensure program success rate stays above 80% by successfully addressing client needs and seeking appropriate services to combat barriers

Provide information, referral, and advocacy services to clients on caseload, and others as appropriate.

Maintains confidential case notes and files for all clients in a timely manner

Input and manage data in systems such as HMIS, Sales Force, and others.

Determine client eligibility for AFC services

Intervene in crises with individual clients and in the general population of the facility

Provide accountability and enforce program expectations to clients

Attend and participate in staff meetings (each Monday); Seek input from team and from Program Director, when appropriate

Assist with program development and policy procedures

Ensure compliance with all relevant laws, including fair housing laws and confidentiality and privacy requirements

Respond to after-hours emergencies, as needed

Skills & Abilities Required:

Bachelor’s Degree in Human Services, Social Work, Counseling

o Or, at least 5 years of experience working in the field of case management, human services, crisis prevention

and/or housing or similar field

Excellent verbal & written communication skills

Excellent computer skills- email, data entry, MS office, and Salesforce (or similar program) experience a plus

Excellent interpersonal skills and ability to positively interact with individuals of diverse backgrounds

Ability to work with a team and collaborate with other staff

Excellent time management skills and ability to work with frequent interruption and often in crisis situations

High level of professionalism

Ability to maintain accurate and timely records

Ability to address conflict and manage crisis situations

Desired Qualifications:

Education (e.g. Chemical Dependency Professional) or Background working in chemical dependency is useful

Bilingual Spanish/English is a plus

Experience in working with people in crisis is preferred

Familiarity with Fair Housing Laws and community resources is highly desired
