If hired for this position, you will become a SWHR CIN employee rather than a Texas Health or UT Southwestern employee.The address is 8150 Brookriver Drive, Dallas, Texas 75247 located at the SWHR Corporate Office.Salary range is Min $15.70 - Max $24.31 – based on relevant experienceWork ScheduleMonday-Friday 8am-5pmJob DescriptionSOCIAL SERVICES TELEPHONIC PATIENT SUPPORT:

Supplies appropriate resource information and coordinating referrals as necessary. Conducts referral researches for complex needs.

Completes applications all agency, county, state, federal programs w/ patient/caregiver, including the following: pharmacy assistance programs, food stamps, Extra Help for prescription costs, Lite Up Texas, transportation, Medicare Savings Program for Medicare Part B, Meals on Wheels/Senior Centers/Companion Services, etc.

Follows-up with clients and correspondence regarding community resources and navigation, applications, and benefits.

Documents all encounters with patient in applicable systems and/or enters all referral activity into systems as dictated by policy/procedure.

Performs other duties as assigned by director, manager or senior staff.

ADMINISTRATIVE SUPPORT:

Provides Care Manager administrative support working under the supervision of licensed staff, assists with daily tasks as needed.

Conducts referral researches for complex needs.

Sends letter to patients per Social Worker, makes social services care plan compliance maintenance calls providing ongoing follow-up and basic motivational interviewing.

Sends timely correspondence to appropriate Care Manager regarding any barriers or problems resulting in the referral/task not being complete and/or a result of a crisis or identified patient need due to their outreach call.

Completes all administrative functions associated with referral activities in a timely manner.
