Hospice Supervisor Schuylkill area locationJob Summary:The Patient Care Manager, Hospice Schuylkill Area is a skilled Nurse leader tasked with direct patient care and [, accountability within assigned hospice teams in the Schuylkill area.With the Director, the PCM assumes 24-hour accountability for the direction and management of patient care within assigned hospice teams. In conjunction with Director and other members of the hospice leadership team, manages material, financial and human resources within the assigned hospice program insuring exceptional end of life care 24 hours daily, seven days per week for all patients under the care of the hospice team. Assures and/or provides patient care in accordance with the Hospice RN job description.Responsibilities:Assesses, plans, implements and evaluates the care for designated patientsEffectively oversees administrative and operational issues for the designated care delivery teamUses effective and appropriate communication styles with patients, significant others, visitors, staff and professional colleaguesFunctions as a department team leader to support hospice and hospital goals and objectivesParticipates in and applies research in practiceMinimum Requirements: Graduate of an accredited school of nursing. Baccalaureate degree in nursing. Individuals selected for this position who do not hold a baccalaureate degree are required to actively pursue and complete the same within the defined timeframe.Minimum three years' experience as a registered nurse in a hospice.week for all patients under the care of the hospice team.
