Coordinates a variety of services including crisis intervention, bereavement, client advocacy, referrals, prevention, education, discharge planning and implementation, and psychosocial assessments. Collaborates in the development and implementation of performance improvement activities.Minimum Requirements: MSW. One year experience in a medical/clinical setting.Preferred Qualifications: MSW, LSW preferred but not requiredThree years experience in a medical/clinical setting.Bilingual preferred, if defined population requires.Licensure and Certifications: LSWMasters in Social Work (MSW) Degree from a program accredited by the council on Social Work Education;Case Management Certification required with in 3 years of hire.Influenza vaccination is required at most LVHN facilities for all colleagues with patient contact unless medically contraindicated or due to a bona fide religious belief or practice, and is strongly encouraged for all colleagues.
