Care Coordinator/QMHA

This full-time, hourly, Bachelor's level Care Coordinator position a LifeWorks NW multi-service site located in SE Portland (Rockwood area).

The Care Coordinator position provides a wide range of clinical and administrative services to improve client care within the medical and psychiatric setting; to assist in the coordination and linking of clients to medical and psychiatric services; works to optimize service delivery by assisting with and facilitating improved access to clinical care across agency programs as part of a strengths-based, consumer-centered approach.

Days/Hours: This is a full-time position working 40 hours a week. Monday through Friday, 8:00 am to 5:00 pm.

Qualifications:

- Bachelor's degree in a behavioral health field (Psychology, Counseling, Social Work, etc.) and/or three full years providing mental health related services.

- This position requires a valid and active QMHA credential certification through MHACBO upon hire.

- The CADC credential is desired given the high occurrence of substance use/abuse.

- Preferred: Clinical experience providing outpatient services or working in community health, primary care is preferred.

- Demonstrate effective communication skills and ability to provide culturally-competent treatment services to diverse client populations.

- Good computer skills including a functional knowledge of Microsoft Outlook and Word. Ability to master proprietary software including electronic health record system.
