Our team is always growing.We are currently hiring for a Health Homes Care Manager in our Rochester Area! The Health Homes Care Manager is responsible for the coordination of all medical and psychosocial care for the Health Homes consumer. The Care Manager establishes relationships with the consumer, his/her supports and all service providers in order to work towards ensuring the consumer’s needs are being met.CORE RESPONSIBILITIES:

Utilize various means of outreach to initiate contact with individuals referred for services, including but not limited to conducting and participating in different community outreach events

Demonstrate thorough understanding of Health Homes program and services

Screen individuals to verify eligibility for services

Identify individuals’ formal and informal support systems that may impact care

Develop relationships with community resources

Assess to identify problem areas to assist in case management care plan

Assess immediate needs, develop and implement action plan if necessary

Maintain caseload of approximately 50-60 clients with a minimum of one monthly contact with each. This can include family members and service providers.

Provide comprehensive care coordination and treatment planning

Participate in transitional care from inpatient settings to community, including post discharge follow up

Coordinate with service providers and informal supports involved in care

Knowledge of community resources including Social Services and SSA

Maintain documentation of outreach process for each client, including but not limited to progress notes, assessments and contacts with all service providers

Perform other duties, which may, from time to time, be assigned

QualificationsEDUCATION AND EXPERIENCE

Bachelor’s Degree in Social Work, Psychology, Sociology, Counseling, Rehabilitation or other Human Services related field with at least 1 year experience providing care management related activities preferred.

SPECIALIZED SKILLS:

Ability to manage time management efficiently

Excellent written and verbal communication skills

Demonstrate ability to interact with community providers and colleagues professionally

Proficiency with Microsoft Office and GMAIL

Ability to meet and maintain deadlines

MISCELLANEOUS:

Must possess clean NYS driver’s license and reliable transportation
