- Maintains a current state license as Licensed Practical Nurse
- Supervisory and management experience
- Ability to handle multiple priorities

- Excellent verbal and written communication skills
- Has good organizational and time management skills
- Experience with seniors preferred
- Possesses the ability to effectively read, write, and communicate in English
- Able to make responsible choices and decisions and act in a resident’s best interest
- May require lifting or bending. Must be able to lift 25-35 pounds
Director of Wellness
Supervisory/Exempt- Recruits, hires, supervises, and coaches all personal care workers in the home
- Completes a 90-day performance reviews and all subsequent annual reviews

- Review personal history and Personal Service Assessment by Wellness nurse
- Create Activities of Daily Living (ADL)
- Schedule shower and Laundry days
- Assign and discuss new care plan with the personal care aidsReview ADL documentation for completion and signatures
- Ensure appropriate staffing scheduling and completion of daily assignments.
- Assists residents with activities of daily living, including bathing, dressing, grooming, toileting, transferring and getting to and from activities and meals according to the personalized service plan.
- Communicates regularly with the Wellness Director.
- On-call 24 hours for staff call-outs, scheduling and questions
- A Case Management record shall be maintained on all residents. Initial documentation upon admission, 30 days, quarterly and annually thereafter
- Participate in the interviewing and hiring process.
- Participate in the ongoing structured orientation program for all new personal care aids employees.
- Conduct a 90-day performance review and make recommendations for continued employment
- Provide regular, on-going training as needed.
- Maintain budgetary guidelines for daily staffing hours
- Provide daily coaching and support to personal care aids
- Lead by example, being visible, involved and accessible
- Complete staffing schedule in a timely manner according to budget guidelines
- Complete and submit staffing hours for payroll
- Complete timely performance reviews.
