Summary: The Resident Care Coordinator/LPN is responsible for the overall personal care services in the home. Responsibilities include, but not limited to hiring and training of the personal care aids (HHA's); annual review and ongoing coaching and counseling of the staff members: completion of Activities of Daily Living, staffing schedule and submitting payroll hours. Ensure that the universal worker concept is implemented. As the leader of the resident care services expectation is to develop and maintain a positive atmosphere for residents, families, visitors and staff. Enjoy a Tuesday through Saturday work week with flexibility in a supportive, intimate work environment.

Qualifications:

  • One (1) year experience in long term care or assisted living
  • Maintains a current state license as Licensed Practical Nurse
  • Supervisory and management experience
  • Ability to handle multiple priorities
  • Able to direct the work of others
  • Excellent verbal and written communication skills
  • Has good organizational and time management skills
  • Experience with seniors preferred
  • Possesses the ability to effectively read, write, and communicate in English
  • Able to make responsible choices and decisions and act in a resident’s best interest
  • May require lifting or bending. Must be able to lift 25-35 pounds

Supervisory/Exempt

  • Recruits, hires, supervises, and coaches all personal care workers in the home
  • Completes a 90-day performance reviews and all subsequent annual reviews

Resident Care and Programming

  • At time of move-in;
  • Review personal history and Personal Service Assessment by Wellness nurse
  • Create Activities of Daily Living (ADL)
  • Schedule shower and Laundry days
  • Assign and discuss new care plan with the personal care aidsReview ADL documentation for completion and signatures
  • Ensure appropriate staffing scheduling and completion of daily assignments.
  • Assists residents with activities of daily living, including bathing, dressing, grooming, toileting, transferring and getting to and from activities and meals according to the personalized service plan.
  • Communicates regularly with the Wellness Director.
  • On-call 24 hours for staff call-outs, scheduling and questions
  • A Case Management record shall be maintained on all residents. Initial documentation upon admission, 30 days, quarterly and annually thereafter

Hiring & Training

  • Participate in the interviewing and hiring process.
  • Participate in the ongoing structured orientation program for all new personal care aids employees.
  • Conduct a 90-day performance review and make recommendations for continued employment
  • Provide regular, on-going training as needed.

Budget and Staffing

  • Maintain budgetary guidelines for daily staffing hours

Administrative

  • Provide daily coaching and support to personal care aids
  • Lead by example, being visible, involved and accessible

Leadership

  • Complete staffing schedule in a timely manner according to budget guidelines
  • Complete and submit staffing hours for payroll
  • Complete timely performance reviews.
