Provincetown, Wellfleet & Harwich Port: Positions available at each health center site ESSENTIAL JOB FUNCTIONS Summary: Recovery Coaches are fully integrated team members who identify as a former recipient of recovery services & now use that experience to provide individual & group support to people receiving or seeking treatment. Recovery Coaches assist in connecting people to resources, supports, & opportunities in the larger community outside of the mental health system. Recovery Coaches contribute expertise & consultation to the entire health center team to help create a culture in which the point of view & preferences of each person we support are recognized, validated, & integrated into all services of community recovery activities. Developing & implementing a flexible but specific work plan that supports the management & triage of multiple priorities & needs, while ensuring adherence to a treatment timeline. Connect with new participants to identify needs & develop a wellness plan. Assist in developing & sustaining natural supports Match the client?s needs with appropriate community resources. Provide peer support to engage participants in a recovery partnership based on the principles of mutuality & services at times coordinated with participants & in publicly accessible locations such as peer recovery centers, restaurants, coffee shops, etc. Adhere to applicable laws, regulations & agency policies governing the disclosure of client identifying data & confidentiality. Attend all mandatory meetings, trainings, group supervisions, & individual supervisions. Track patient progress through contact & ongoing reassessment, collecting & inputting all required data into the required systems (e.g., electronic medical records). Facilitate & coordinate follow-up case conferences with referring agencies. Adhere to established case management standards of care. Coordinate & lead informal groups related to peer support, recovery, resiliency, & resources. Advocate for the people with whom they work, promoting their autonomy, protecting their rights, & providing access to resources, information, skills & supports. Connect & network with the larger community & other area peer support providers for the purposes of developing best practices, gathering community resources & mutual support. Experience with evidence-based screening & assessment instruments desirable. High school diploma or GED certificate. Post-secondary education in social work or psychology preferred Recovery Coach certification a plus. CPR & First Aid certification a plus Experience in the profession of mental health, substance use, trauma, & related social services. Case management, navigation, or community health worker experience desirable. Must have the ability/experience to work positively with a wide range of people & partnering organizations, both inside & outside of Outer Cape Health Services, as well as the ability to manage multiple tasks & projects, & handle stress in a positive way. Experience with eCW or other electronic medical records system desirable. Must have experience in addiction recovery services. Valid driver?s license & reliable transportation.
