Job Description

Phoenix Home Care is currently seeking an Intake Coordinator for our Home Health Dept.

POSITION SUMMARY

Responsible for data entry, order tacking, file uploading, phone triage, and general office duties.

DUTIES AND RESPONSIBILITIES

  • Communicates with clinical staff as needed to clarify data entries.
  • Phone contact of all new patients and hospitalizations.
  • Assists in client requirements and reporting as needed.
  • Tracking, faxing and attaching all physician orders and lab results.
  • Light IT duties with field tablets.
  • Performs general office duties as assigned.
  • Performs other administrative duties as assigned.

MINIMUM JOB REQUIREMENTS

  • Education/Licensure: LPN valid in Kansas
  • Experience: One (1) year of office/clerical experience preferably in a home health agency.

KNOWLEDGE, SKILLS AND ABILITIES

Knowledge of medical terminology. Computer/word processing skills desired. Ability to meet minimum agency hiring standards.
