Large non-profit organization looking for Case Manager to work with their Essex County Re-entry Vocational Training Program. Job Description: The Case Manager seeks qualified client referrals from Essex County Sheriff?s Department facilities. Provide continuous advocacy and education around supports and services in the community in order to reduce recidivism, homelessness, and advocate employment. Attend train-the-trainer C-Tech training during the orientation period. Requirements: Bachelor?s degree in education, psychology, rehabilitative counseling, or related human services field and one year of experience in providing intake and outreach activities including home visit services and case management, or equivalent Demonstrated experience/work history working with housing authorities, employers and employment placement agencies, mental health clients and substance abuse programs. Reliable transportation and excellent driving record Key Activities: Participate in outreach and advocacy efforts to help inmates in Essex County. Develop, maintain, and document a strong network of referral sources designed to generate positive outcomes for program participants. Connect with other providers, employers, and employment placement agencies, including housing providers, for referrals and linkages. Provide pre-screen, intake, and assessment on qualified participants, and work with community partners to attain housing, employment, treatment, and other supportive services. As C-Tech trainer, provide instruction to participants and evaluate performance. Provide day-to-day support and services for participants with a specialization in employment placement and support, homelessness prevention/rapid re-housing and transitional supportive services. Assist the prospective clients in gathering documentation necessary for enrollment. Conduct regularly reviews referral log to determine applicant trends and percentages of applicants accepted to and attending the program.
