Good Samaritan Home Care’s quality care is provided through a team of registered nurses, social workers, aides and physical, occupational and speech therapists.Responsibilities
Coordinates care with caregivers and physicians to improve and/or maintain the social, emotional, functional, and physical health status of the patient.
Assists the physician and other caregivers in understanding the significant social, emotional and economic factors influencing the patient's ability to improve medically.
Provides resources and education to patients and their caregivers
Participates in the coordination of services with other disciplines and community resources as needed
Evaluates and revises care plans/assessments based on changes in patients and/or their environments
Participates in regular case conferences and in-services or special case orientations as needed
Qualifications
Master’s degree from an accredited school
Currently licensed to practice in the state of Indiana
Must obtain and maintain current CPR Certification (American Heart Association or American Red Cross)
Valid drivers license, car insurance and reliable transportation
One year of social work experience in a health care setting required
