OverviewTHIS POSITION IS LOCATED AT 540 N. Alabama Indianapolis, IN.Provides case management and care coordination services focused on enabling residents to live independently and assume full responsibility and control of their lives; provides counseling services to assist residents, including adults and children, with adjustment to residential living; through individual and group sessions, assesses resident's needs (including adults and children) for additional services. Services to include street outreach and continuing support services.Responsibilities1) Develops case plan for each resident within 2 business days of admission to Center; case plan to include planning including but not limited to the following areas: a) Housing; b) Employment and job readiness; c) Legal services; d) Personal finance; e) Public assistance; f) Health, including mental health; g) Child care; h) Transportation; i) Material and financial assistance; j) Enhancement of family functioning; k) Refers Spiritual concerns to Corps officer. l) Outreach services m) Continuing support services2) Assists residents in implementing case plan by regular (at least 3 to 4 times per week) individual counseling sessions focused on: a) Providing information concerning community resources; b) Encouraging and following up with residents to implement details of case plan; c) Encouraging and assisting clients in following rules in the Center Handbook; actively functions as mediator in situations of conflict.3) Functions as a liaison on behalf of resident with other agencies; functions as advocate for residents and clients of the Center as needed.4) May be asked to lead educational, instructional or support groups in one or more of the following areas within the Center: a) Domestic violence support; b) Women in Recovery; c) Children’s' groups; d) Community Resources group.5) Facilitates residents' adjustment to Center living by: a) Initiating and maintaining communication with all Center staff on issues relating to adjustment of residents' to the Center; b) Ensuring adequate communication of Center's expectations of residents; reads Handbook to residents if needed; c) Function in partnership with Child Care staff to enhance positive behavior and adjustment of children to the Center. d) Function in partnership with Advocates to enhance and ensure stability and safety to consumers in shelter.6) May be asked to conduct educational, instructional and support groups in one or more of the following areas in the community: a) Domestic violence awareness; b) Prevention of homelessness.7) Offers Life Skills training for all residents for an hour each day; a) Plans, researches, teaches topical information, b) Coordinates outside resources (speakers, videos, etc).8) Maintains records and keeps statistical reports as required by The Salvation Army and other systems.QualificationsMinimum education of an Associate’s Degree along with plan towards Bachelor’s Degree in a social work or other human services field; Bachelor’s Degree preferred; experience in a social service setting providing services to persons in crisis, homeless persons, domestic violence or mental health preferred.Job LocationsUS-IN-IndianapolisJob ID2018-8341of Openings1CategorySocial ServicesTypeRegular Full-Time
